Please note:
A list of in-network insurance plans is available here. You, as a patient, are responsible for providing accurate insurance and personal information, including any preferred laboratories for pathology or bloodwork, or other testing.
Payment is expected in full at each visit if you are not insured by a plan that we are contracted with.
You must bring an up-to-date insurance card and government-issued photo ID to your appointment.
You must pay all co-payments and deductibles at the time of service.
If your insurance plan requires a referral authorization from your PCP or a pre-authorization from your insurance, you will need to contact your PCP or insurance company to be sure it has been obtained.
